Diagnostic error is the third most frequent malpractice allegation in the outpatient setting and leads all other allegation categories in total costs expended (MMIC data). Fortunately, there are many points in the diagnostic process on which to focus improvement efforts, from the initial assessment (where half of issues arise) through testing and results processing, to following up with patients and coordinating subsequent care. Common pitfalls include inadequate follow-up on test results, disruption of referrals, and failure to report incidental findings — evidence that meaningful improvement is best achieved through a team-based, systems-focused approach rather than a too-narrow emphasis on the physician.
