Constellation Privacy Notice

We are committed to safeguarding privacy and the confidentiality of your personal information. This Notice describes how we collect information, how we use this information and the circumstances where information may be disclosed to third parties.

This Privacy Notice applies to Constellation and Constellation’s and its subsidiaries’, including, but not limited to MMIC Insurance, Inc., UMIA Insurance, Inc., Arkansas Mutual Insurance, Inc. and InsureCAREteams™, a product of UMIA Insurance, Inc. (collectively, “Constellation,” “us,” “our,” or “we”) websites, portals, products and services that collect data and display these terms (collectively, “Sites”).  This Privacy Notice does not apply to Constellation Sites, or its subsidiaries that do not display or link to this statement or that have their own Privacy Notices.  

We do not knowingly offer or sell products or services to children.  Our Site is not intended for anyone under 13 years of age. 

We do not offer or sell products or services outside the United States.

By using this Site, you agree to the terms and conditions contained in this Privacy Notice and the Conditions of Use and/or any other agreements that we might have with you.  If you do not agree to any of these terms and conditions, you should not use this Site or any Constellation or Constellation Members’ Site or any products and services.  Please read the Terms of Use before using our Site or submitting personal information to us.


We will only collect and use your information to provide the services you purchase from us and in accordance with applicable privacy laws.

Information you provide

When you are our customer or a potential customer, we ask you to share information so we can provide you with the desired products and services.   We collect your personal information when you complete an application for coverage.  This may include, but not be limited to:

  • Full name (first, middle and last name)
  • Address
  • E-mail address
  • Telephone number
  • Social security number
  • Date of Birth
  • Licensing or regulatory board actions
  • Professional license information
  • National Provider Identity (NPI) number

You may also be required to provide certain other information for underwriting purposes or to access and use certain products or services that we offer.

We collect and use this information to provide you with products and services including:

  • Underwriting
  • Arranging and administering insurance
  • Risk analysis mitigation
  • Managing claims, including investigation and settlement
  • Accounting and auditing
  • Responding to your inquiries
  • Other products or services that you purchase or that are provided to you through your policy

As our customer, you may receive communications related to your policy or coverage and other types of communications, such as newsletters and announcements about products and services.  You can manage your email subscriptions by subscribing or unsubscribing at any time.

Your correspondence with us

If you correspond with us by email, postal service or other forms of delivery, we may retain this correspondence and the information contained in it to respond to your inquiry, notify you of Constellation services, or keep a record of your question or request.   

We will use the email address you provide when creating an account or when submitting an application for coverage.  We will maintain your username and/or email address and password when you access your account via the Site.

Payment card information

If you choose to purchase products and services from Constellation you will be sent to our partner’s website for credit card payment processing.  

Constellation does not view, collect or retain your credit card or payment information anywhere on our network.

Information we get from third parties

We may collect information from your insurance broker or agent, or your employer if you have authorized them to provide it to us.

This information may be collected using various means including, in writing, in person, by telephone, by email or through other electronic vehicles. 

When you, or someone you authorize, complete an application form to purchase an insurance policy, we may collect additional information from publicly available sources.

If you chose not to provide your information

Without your personal information, we will not be able to provide insurance coverage, administer your policy, answer questions or process a claim. 

You may still use portions of the Constellation site without providing us your information.  Some personal information is necessary to create an account or choosing to subscribe to other services or products.

We will only collect information for the purposes outlined above.


Information you provide

We collect information you choose to share with us.  For example, we collect information when you: request a quote for insurance; complete an application; submit information about a potential claim; register and create a username and password for our customer portal; and communicate with us or request information/ materials.

We do not require you to register or provide this information to us to view our general site.

Information collected automatically

Like other websites, we collect certain information automatically when you access our Site. Constellation uses a web analytics service provided by Google, Inc. (“Google”).  Google Analytics uses ‘cookies’, to help us analyze Site use.  The information generated by the cookie about your use of the Site (including your IP address) will be transmitted to and stored by Google on servers in the United States. We receive a report with aggregate information that we use to evaluate Site use and activity. Google may also transfer this information to third parties where required to do so by law, or where such third parties process the information on Google’s behalf.  Google will not associate your IP address with any other data held by Google.  By using our Site, you consent to the processing of data about you by Google in the manner and for the purposes set out above.

Constellation may track aggregate data about users who log into the secure portal section of our Site and their navigation patterns.  Constellation does not track customers when they cross to third-party websites and does not provide information for third-party advertising to them.

Constellation uses a marketing database management program that deploys a cookie when a user interacts with a marketing or customer communication, such as an email announcement or a marketing-based landing page on the Site.  This cookie collects personal information such as your name, email address, if you accessed a link, which pages you visited on our Site, your history arriving at the Site, your purchases and the like.  We use this information to evaluate the effectiveness of our marketing campaigns.  You may set your browser to block these cookies.

You are always free to block, delete, or disable these technologies if your browser, application or device allows. However, if you decline cookies or other similar technologies, you may not be able to take advantage of certain Site features or tools. This site does not support the Do Not Track browser settings


As explained above, Constellation processes your data to provide you with the products and services you have purchased or requested from us, including your insurance policy.  We use this information to administer your policy and for managing or processing claims.  We use your information to improve our products and services to better tailor them to your needs, to communicate with you about your policy, to investigate claims and to provide other policy-related information.


Constellation does not sell your personal information.  We may disclose your information only as described here.

We may make your personal information available to third parties as permitted by applicable law to administer our insurance policies or when required by law. We may provide your personal information to:

  • Contractors or third parties providing services to us related to the administration of insurance policies, such as: distributors, agents, claims and loss assessors, claims managers, and insurance reference bureaus.
  • Related bodies and corporate companies in the Constellation, Inc. insurance group that provide administrative services, process insurance policies, administer claims and submit required regulatory reporting.
  • Third-party administrators, emergency providers, repairers and suppliers, and investigators for the purpose of processing and administering your claims.
  • Governmental authorities, law enforcement agencies and dispute resolution bodies and agencies.

In addition, we may disclose information where we think it is necessary to investigate or prevent an actual or suspected crime or injury to ourselves or others, if required by law, or in response to a request from law enforcement authorities or other government officials.

During the normal course of our business, Constellation may sell or purchase assets. If another entity acquires, or seeks to acquire, Constellation or any of its assets, information we have collected about you may be transferred to such entity. In addition, if any bankruptcy or reorganization proceeding is brought by or against us, such information may be considered an asset of ours and may be sold or transferred to third parties. Should a sale or transfer occur, we will use reasonable efforts to require that the transferee use personally identifiable information provided through our Site in a manner that is consistent with this privacy statement


We maintain administrative, technical, and physical safeguards and require our third-party partners to meet acceptable privacy and security standards. We restrict access to your personal information to those workforce members who need to know your information in order to provide services to you.  We provide ongoing training to our employees and regularly test our security technology. 


We store your personal information on various internal systems and databases, including; shared drives, email, document management systems, hard copy, and on the servers of the cloud-based database management services that Constellation engages.

Constellations retains your information throughout the course of your business relationship and then as required to meet applicable law.

  • Full name
  • Policy number
  • Details of the request for correction
  • If you are seeking access, view or correct information on another person’s behalf, we also require written authorization from that individual.


Cookies can be disabled or removed by tools that are available in most commercial browsers. The preferences for each browser you use will need to be set separately and different browsers offer different functionality and options.

Constellation does not sell the information gathered through cookies.


This Privacy Notice may change, and Constellation reserves the right to amend this Privacy Notice and Conditions of Use at any time, for any reason, without notice to you, other than the posting of an amended Privacy Notice and Conditions of Use on this Site.
