Constellation® is pleased to co-sponsor the PACT Collaborative: a breakthrough series Collaborative to create comprehensive, highly reliable Communication and Resolution Programs (CRPs). The Collaborative will be led by Ariadne Labs, Institute for Healthcare Improvement and the Collaborative for Accountability & Improvement at University of Washington School of Medicine.
Over 18 months, this Collaborative will bring CRP experts together with health care clinicians and teams ready to improve and champion CRP programs within their own organizations. Collaborative participants will receive tools and resources around the key elements of successful CRPs:
